A Caboolture toddler who died after suffering traumatic injuries should not have been returned to his family.

Mason Jet Lee was found dead on June 11 after suffering severe traumatic injuries which doctors believed were inflicted over a long period.

The 21-month-old had been admitted to hospital in the months before he died. Doctors reported the risk to the child safety department and fully discharged him, reports 9 NEWS.

The state government today said it had opened a review into the actions of doctors and health workers in the lead up to Mason’s death.

The state’s opposition has described the state as the “worst” in the nation when investigating child abuse.

Shadow Child Safety Minister Ros Bates said Mason’s death was due to “a broken system”.

Addressing media this morning alongside Opposition Leader Tim Nicholls, Ms Bates said the child safety system was in “crisis”.

Ms Bates has called on the Palaszczuk government to “explain to Queenslanders what her department did, what they knew and what they’re going to do to make sure what happened to Mason never happens again”.

“There is another Mason out there,” Ms Bates said.

Mr Nicholls said the government had put “no new staff” onto the child safety protection area in the latest budget.

Health Minister Cameron Dick said initial investigations showed the clinicians who saw Mason did “what they needed to do”.

“Let’s look at all parts of the system to see what has occurred and what we can do to learn from this,” Mr Dick said.

Mr Nicholls said the government had been warned in October that the department was “breaking at the seams”.

“It seems to be a system in crisis, breaking at the seams,” he said.

A police homicide investigation into Mason’s death continues.

  • Heartbreaki g indeed !! There’s no justice served for this little man ! There are too many kids out tgere who daily live in danger ! It’s terribly sad that the system is failing indeed, not only in Australia, in other countries as well… ????????

  • What a tragic devastating story, I believe the judge is right and the system failed that young child. It’s a shame that there are limited places for government services to place children.

  • It appears useless reporting matters to child protective services – they fail children time and time again. There has to be a better system.

  • the system is screwed up, i called to report an abuse of a child by their mother, they told me if the police did nothing there was nothing to worry about, idiots, she was charged with assault later!

  • It appears all states are in dire need of improving their services. There definitely isn’t enough staff to handle the cases. There also needs to be enough fostering resources too. In some cases there needs to be checks on them too. I know of a case where children were being fostered interstate following the Mum taking them there and the partner not wanting them. The children were mistreated by the foster parents. Clothes sent to the children for birthday presents were given to their own children and the foster children got the old clothes of their own kids. Relatives in their home state were contacted an asked if they would have them. The foster parents in their home didn’t have enough furniture nor could they afford to so they were paid an allowance to buy basic furniture for the kids. When they left the furniture went with them.
